A Comprehensive Approach to Managing Hospital Readmissions
Tuesday March 13, 2:00-3:30pm
Studio 9 & 10
Value-based purchasing (VBP) penalties and incentives for skilled nursing facilities begin October 1, 2018. The need for a comprehensive approach to managing hospital readmissions is greater now than ever and as the medical complexity of residents continues to increase a programmatic approach becomes the standard of practice.
The successful management of hospital readmissions is dependent on a multifaceted approach to clinical care. This session will focus on several approaches that are concerned with the provision and evaluation of nursing care. The first approach is assuring the staff has the nursing competencies to manage the care, the second is providing care that is founded in evidenced based practices, and the third is evaluating the care processes through quality assurance and performance improvement. This interactive session will take you through the process of recognizing the components of a nursing competency program, identifying the best practices associated with managing hospital readmissions and discuss the role of QAPI in the evaluation of the effectiveness of your clinical processes.
