Collaborative Health and Wellness

Health and wellness is a team sport. Post-acute care providers are in a unique position to address the needs of their patients and provide a smooth transition back to the community. CareCommunity gives you the tools to collaborate as a patient, a family member, and a healthcare provider.

It is the personal team of family and friends that is essential to support each of us. It is also the team of professionals that provides care in individual settings; the extended teams that coordinate services across care settings; and the larger communities in which we live.

We begin by first identifying who is on our team. To support care across settings, we invite individuals from all of their usual roles and organizations.

These teams need information and tools to work well together. This includes bringing in information from encounters with the healthcare system (the Care Summaries produced by each of them). The information is then reassembled so that it can be viewed over time, across all care settings. This process creates the history of what has happened.

We also want a plan for what is being done to regain health and then remain healthy. This is a combination of:

  • personal activities (diet, exercise, taking medications, medical and other appointments),
  • tracking of health status (how we feel, our weight, the amount of exercise we’re getting, other tests that are being performed)
  • responding to alerts about changes detected by the system

Healthcare may be a critical step in our health, the heart surgery or joint replacement that makes it possible for us to get on the road to recovery. Then we need to be in a supportive environment that keeps us safe and allows us to access the services we need, from groceries and pharmacies to rehab and transportation services. Without those services, without our social network that keeps us motivated, it’s all the harder to take on the behaviors that we need to be healthy. All these elements work together. CareCommunity is the place where this all comes together.

Larry Wolf

Larry Wolf joined MatrixCare as Chief Transformation Officer in September 2017. With more than 40 years of industry experience, Wolf uses his provider and health IT background to help communicate the company’s value proposition in a way that resonates with customer needs and vantage points. Prior, Wolf held leadership roles at Kindred Healthcare, including CIO for Emerging Markets. He is chair for the Health Information Management Systems Society’s Public Policy Committee (HIMSS PPC) and a member of the LTPAC Health IT Collaborative. He holds a BS in Language & Communication from Hampshire College in Amherst, MA and a master’s in Computer and Information Sciences from the University of Massachusetts.
