INTERACT Helps Improve Quality Outcomes & Reduce Hospital Readmissions
Interventions to Reduce Acute Care Transfers (INTERACT) is a quality improvement program that aims to improve the identification, evaluation and documentation of acute changes in resident status and condition in long-term care facilities. It involves a collection of communication, care pathway and care planning tools that, when used in concert, can reduce the likelihood of rehospitalization.
INTERACT tools are available within the SigmaCare by MatrixCare software to assist in reducing rehospitalization.
- Stop & Watch: CNA’s are often the first to notice subtle changes in a resident that could indicate a larger problem. Completing the brief Stop and Watch tool will alert the nurse to these changes.
- SBAR: Upon evaluation of the reported changes on the Stop and Watch, the nurse may need to contact the physician. The SBAR is a tool designed to capture all of the information that the physician may need to make a well informed decision on the follow up course of care.
- Change in Condition Cards: Answers provided by the nurse in the SBAR tool will trigger notifications which will provide guidance on the urgency of physician notification.

2020 ICD-10-CM Information Available
CMS has published information on the ICD-10-CM updates for FY 2020. The 2020 ICD-10-CM codes are to be used for discharges occurring from October 1, 2019, through September 30, 2020, and for patient encounters occurring from October 1, 2019, through September 30, 2020.
CMS Training Available for Coding Specific Section GG Items
The Skilled Nursing Facility PPS Patient Driven Payment Model (PDPM) relies on accurate coding of several MDS Section GG mobility and self-care items that impact the PT, OT, and Nursing component per-diem case-mix payment rates.
On June 7, 2019, the Centers for Medicare & Medicaid Services (CMS) released a series of short videos to assist providers with coding select Section GG items on the MDS. These videos, ranging from 4-12 minutes, are designed to provide targeted guidance using simulated patient scenarios. To access the videos, click on the links below:

CDC Infection Control Training and Education
Environmental services (EVS) personnel have the shared responsibility to help stop healthcare-associated infections (HAIs) from spreading by working with colleagues, prioritizing areas that pose immediate safety risks, and following guidelines for cleaning and disinfecting. An interactive training is now available which illustrates the important role EVS personnel have in the prevention of HAIs.
