INTERACT Helps Improve Quality Outcomes & Reduce Hospital Readmissions

Interventions to Reduce Acute Care Transfers (INTERACT) is a quality improvement program that aims to improve the identification, evaluation and documentation of acute changes in resident status and condition in long-term care facilities. It involves a collection of communication, care pathway and care planning tools that, when used in concert, can reduce the likelihood of rehospitalization.

INTERACT tools are available within the SigmaCare by MatrixCare software to assist in reducing rehospitalization.

  • Stop & Watch: CNA’s are often the first to notice subtle changes in a resident that could indicate a larger problem. Completing the brief Stop and Watch tool will alert the nurse to these changes.
  • SBAR: Upon evaluation of the reported changes on the Stop and Watch, the nurse may need to contact the physician. The SBAR is a tool designed to capture all of the information that the physician may need to make a well informed decision on the follow up course of care.
  • Change in Condition Cards: Answers provided by the nurse in the SBAR tool will trigger notifications which will provide guidance on the urgency of physician notification.

Please contact your Account Manager to discover more about implementing this module at your facility.

Increase Census & Maximize Reimbursements with ReferralConnect

Today’s complex healthcare environment is more challenging than ever. With reimbursement changes and narrowing referral networks, it’s becoming more difficult for providers to increase revenue and optimize census. Designed specifically for long-term and post-acute care providers, ReferralConnect delivers robust tools and intelligence so you can discover hidden referral sources, focus your marketing efforts, and build sustainable relationships that attract even more referrals.

Qualify Referrals to Ensure Quality Outcomes
Capture clinical-based information to aid in qualifying referrals prior to admission. Leverage the information tracked for internal analysis and subsequent conversions with referral sources. Admitting the residents that best fit your facility’s care capabilities will be critical to maximizing reimbursements under PDPM.

Measure & Manage Referral Sources to Optimize Census
Qualify the true potential of all the new and existing referral sources so you can prioritize your resources on those that are high value. Monitor activity to provide visibility into referral source relationships and identify referrals from that referral source. Analyze market and referral data to identify the best sources of referrals that best fit your ideal resident profile and lead to more admissions.
