How a continuing care retirement community connects care across its campus

Stonehill Communities is a not-for-profit organization with a mission to offer dignity, care, and quality of life for older adults.

We provide a continuum of care on and off campus so residents and clients can transition to different levels of care as their needs change.

Our services include independent living, assisted living, assisted living memory care, post-acute rehabilitation, long-term care, outpatient therapy, a Wellness Center, affordable senior housing, home care, dementia care support through the CMS Innovation Center GUIDE Model, and a Caregiver Resource Center for clients in the community.

The challenge

In a community like ours, residents may transition between independent living, assisted living, memory care, rehabilitation, and skilled nursing—sometimes more than once.

To keep care consistent, we need information to move with them. Our teams rely on access to up-to-date clinical information, clear communication across departments, and visibility into each resident’s full history.

That also means reducing duplicate work, streamlining workflows, and making sure leadership can see what’s happening across the community.

The solution

To support a more connected experience across the continuum, we implemented EUID, or the One Resident Record.
